Notice of Rights and Responsibilities (FL–192): If you do not have minor children, you can skip this form.  If you do have minor children, you must attach this form to your FL-180  divorce judgment.  This is a two page form.  The first page describes the procedures you are to follow for reimbursement of the children’s health care costs.  The second page explains that a child support order is part of the judgment and that child support can be modified if significant changed circumstances occur in the future.  The second page also explains how to modify child support.  There is nothing for you to fill out on the FL-192 form.  You just read it, print it, and attach both pages to your FL-180 judgment form.  Click below to download and print a copy of FL-192 so you can attach it to your divorce judgment.
