Income & Expense Declaration

Fill out the FL-150 form available at the bottom of this page, print it, date it, and sign it.  After you have filled out your FL-150, set it aside for now.   Your Income and Expense Declaration will eventually be filed with the court.  The FL-150 will be filed with the court when you submit your Judgment of Dissolution.
