Declaration of Disclosure

Fill out the form at the bottom of this page, print it, date it, and sign it.  Attach copies of your state and federal income tax returns for the past two years.  Then, set it aside for now.  Your FL-140 is not filed with the court. You keep the original and you will eventually give your spouse a copy.  We will discuss what to do with your FL-140 after you have completed the rest of your Preliminary Declaration of Disclosure forms.
